Is this your first time as a patient at Carriero Foot and Ankle? Save yourself some time in the waiting room by filling out the necessary forms beforehand! Please check in online seven days before your appointment and make sure to complete and print the four medical forms in the Patient Portal and bring them with you to your appointment.

Billing
Co-pays are due at the time of your office visit. For your convenience, we accept MasterCard, Visa, American Express, Discover and HSA, as well as personal checks and cash.

Appointment Policy
Please arrive 15-20 minutes prior to your initial appointment and bring your valid insurance card. If you had recent diagnostic studies performed (i.e., X-rays or MRI services), please bring them with you to help expedite your care. Additionally, if you have any written reports from other physicians, please bring them to your appointment.
Save time in the waiting room and complete your patient forms before your appointment.
No-Show and Cancellation Policy
If you wish to cancel your appointment, please do so 24 hours prior to your scheduled appointment. If you fail to appear for an appointment or fail to cancel at least 24 hours in advance for an appointment, you will be billed a “no-show” fee.
Prescription Refills
It is the office policy that medication refills be handled during the hours of 8:30 a.m. to 4:30 p.m. Monday through Thursday. Please allow 48 hours for your request(s) to be completed. Prescriptions cannot be filled over the weekend.
